To ensure a smooth process, we kindly ask patients to bring all relevant paperwork, such as scans and doctors’ letters, to their initial appointment.
In order to secure your appointment, we will need some essential details. Specifically, we’ll require a Letter of Liability, your insurance company information, claim number, and Case Manager details, which can be conveniently submitted via email or fax. This process is crucial for ensuring that your treatment proceeds as planned.
For those with complex cases, multiple areas of concern, or a history of prior physiotherapy treatment for the same injury, we may need additional information.
